چکیده

C oronary artery disease is a worldwide hazard today and stable angina is one of its commonest manifestations. The diagnosis of chronic stable angina is based on the presence of anginal symptoms, a positive exercise test for myocardial ischaemia, and confirmation of coronary artery atherosclerosis by coronary angiography. However, the optimum treatment of these patients has been a controversial issue in recent years. Recent studies, or meta-analyses thereof, have still not fully determined the usefulness of invasive treatment of the coronary arteries with percutaneous coronary intervention (PCI) in comparison with medication. While PCI is known to reduce the incidence of death and myocardial infarction in patients who suffer from acute coronary syndromes, the same benefit does not appear to extend to patients who have stable coronary artery disease. As the availability of non-invasive methods of diagnosing coronary artery disease steadily increases, asymptomatic patients are often referred for PCI. However, a necessary condition for any preventive therapy is that it should combat a threat —which in the case of stable coronary artery disease is acute myocardial infarction and death— with an acceptable cost. Comparison between Revascularization and Optimal Medical Therapy in Patients with Stable Angina Pectoris

Background and Purpose: Regarding usefulness of revascularization versus optimal medical therapy in patients with stable angina pectoris, data are challenging. The aim of this 12-month follow-up study was to compare the survival benefit associated with revascularization versus optimal medical therapy on the patients with stable angina pectoris. Materials and Methods: A prospective clinical s...

QT Dispersion: Does It Change after Percutaneous Coronary Intervention?

BACKGROUND Myocardial ischemia is one of several causes of prolonged QT dispersion. The aim of this study was to evaluate the effect that percutaneous coronary intervention has on the depolarization and repolarization parameters of surface electrocardiography in patients with chronic stable angina. METHODS We assessed the effects of full revascularization in patients with chronic stable angin...

Effects of Asymptomatic Ischemia

Background. Ischemia on ambulatory electrocardiographic monitoring has been shown to adversely affect short-term prognoses in patients with unstable angina, after myocardial infarction, and with chronic stable angina. Methods and Results. In this long-term study, we followed 138 patients (mean age, 59±9 years) with chronic stable angina and positive exercise tests for cardiac events (e.g., deat...

Economic considerations in managing patients with chronic stable angina.

OBJECTIVE To quantify the economic burden of chronic stable angina in the United States, characterize recent trends in the use of coronary revascularization, and compare the clinical outcomes and long-term costs of percutaneous coronary intervention (PCI), coronary artery bypass grafting (CABG), and medical management in patients with stable angina. SUMMARY The direct and indirect costs of st...

Percutaneous coronary intervention in chronic stable angina.

Although the therapeutic advantage of percutaneous coronary intervention in acute coronary syndromes have been proved in numerous studies, its position in the treatment of stable angina remains a controversial issue. The results of the recent studies did not lead into definite answers for the proper treatment of chronic coronary artery disease. The identification of the patients that will benef...
